The heart whispers faint old fears once again!
And these hands dither holding fag ends
Neck shaking stiffens within the membrane
The cortex quivers and a storm, transcends.
With thoughts of will we be lovers or just friends.
With heart palpations to ascertain!
Will you be my one true eternal love Elaine?
Till all the wounds of my heart, and soul amends.
But if not let all men the worlds impends
Be without solace and a lover’s pain
Be without a fortune the redeemer’s fame
Be without destiny and be without, girlfriends.
If it isn’t kismet it’s bad kharma
No need for pilgrimage or the partner.

You don't need the commas in
You don't need the commas in the last lines of the first and third stanzas - I know you're doing it for pacing, but it is grammatically incorrect. It also seems strange when you haven't got punctuation elsewhere where you do need it to help the sense, for example in the first stanza.
Found the last couplet particularly effective, last line is truly excellent.
